The Steady Path Stabilization Center

The Steady Path is a safe place for people who are struggling with mental health challenges to receive professional support in a home-like setting. This 11-bed facility offers around-the-clock care for people who are struggling at home or people who are transitioning from hospital care or jail.

Diversion Care

Hospitalization: One of our goals is to help people before they reach the need to go to the hospital. We offer a place where people with mental health and co-occurring disorders can get care without being hospitalized. 

Incarceration: We work with probation, parole, attorneys, and judges to help people who need mental health care instead of going to jail, especially those who are at risk of being sent to jail because they have struggled with compliance due to their mental health challenges. 

We are a proud partner in the Let Everyone Advance With Dignity (LEAD) initiative that connects those with criminal justice system involvement with resources that can help improve their lives.


 

Step-Down Care

We help people who have been discharged from a hospital but would benefit from extra time and support before going home. We also work with people who have been in jail but would benefit more from mental health treatment than from staying in jail.

This approach provides a safety net, and helps clients reintegrate into daily life and responsibilities at a manageable pace.


The Steady Path is a place where anyone struggling with mental health or co-occurring disorders can feel safe and supported while they work on getting better. Our team, which includes people with their own lived experiences of mental health challenges, helps clients manage appointments for counseling, court, medication, and more. We also help clients find hobbies and activities that support their mental health journey.

The Steady Path provides a welcoming short-term environment and a guided path to wellness to all genders from most Northwest Ohio communities.


A Unique Facility

We focus on helping people right when they need it, often catching those who have fallen through the cracks of traditional systems that didn’t work for them. Everyone who comes to The Steady Path is here by choice, and clients decide how long they stay. Clients don’t pay rent or other fees, we will only bill insurance for the individual mental health care services we provide, like in an outpatient setting. Whether it’s sleeping better, seeing a therapist, or managing mental health medications, our goal is to help clients reach their own self-determined goals.

 



  • One person was struggling with grief after losing a loved one and wasn’t able to take care of themselves or their home. After coming to The Steady Path, they saw a therapist regularly and worked with an occupational therapist to manage their home. Before leaving, they joined a grief recovery group and continued therapy.

  • A case manager working in a jail helped a person who was diagnosed with schizophrenia and was not taking medication. The Steady Path team and the person’s public defender agreed that if they had been getting treatment, they would have been much less likely to be arrested. Together, we worked to get the person out of jail and into The Steady Path program so they could get the mental health care they needed.

  • Another person had been in and out of jail and hospitals for years and had missed out on help from many systems. After a two-month stay at The Steady Path, they were able to manage their symptoms better, connect with others, and eventually move to a group home where they’ve been thriving and even started creating and sharing music online.


RESIDENT SERVICES

CASE MANAGEMENT
Case Managers and Behavioral Technicians from Family Resource Center will help residents with their medical needs, including connections with doctors, dentists, and other specialists when needed. They’ll also offer educational opportunities to help residents stay informed about their health.

Case Managers and Behavioral Technicians will be available to staff for advice and support through internal communication tools.

As part of their roles, Case Managers and Behavioral Technicians will review residents’ medications carefully. If needed, they’ll reach out to primary care doctors, pharmacists, or hospitals to make sure everything is on track.

PSYCHIATRIC CARE
Access to psychiatric care is an important part of the support we offer. Residents can connect with Psychiatric Nurse Practitioners through our telehealth services.

These telehealth services are provided by Family Resource Center's remote team and are available on-site at The Steady Path. Appointments for evaluations and follow-ups are offered Monday through Friday.

If the Care Coordinator or any staff member notices an urgent need for psychiatric evaluation or management, we’ll make sure the resident is added to a provider's schedule as soon as possible.

CLINICAL TREATMENT SERVICES
When residents at The Steady Path are ready to leave the facility, they’ll resume their regular appointments with the clinicians and case managers they’ve already been working with.

Peer support staff play a big role in helping residents stabilize and succeed. That’s why we welcome them to meet with clients at The Steady Path.

While residents are staying at the facility, they’ll have access to tele-counseling services provided by the Family Resource Center’s remote team. These sessions are designed to help with the stabilization process while they’re here, but they’ll stop once residents are discharged.

For residents who need extra support, daily tele-counseling appointments will be available. These can be standard 60-minute sessions or split into two 30-minute sessions to help more people and meet individual needs. The Facility Case Manager or Program Manager will handle scheduling these appointments.

If a resident hasn’t worked with the Family Resource Center before, they’ll be connected with the right providers to start the enrollment process and complete a Comprehensive Diagnostic Assessment.
